Scuba Travel Insurance Questionnaire

Hi all,

I'm conducting some research into the choices people make about their scuba diving travel insurance.

I'd really appreciate it if you could take literally 20 seconds out of your life to answer just 4 quick questions.

This is a non-commercial, research questionnaire. I?m not employed by anyone in the diving or insurance business. I won?t be selling the data, all questions are optional and no personal details will be collected. Unfortunately, you will not be entered into a prize draw or any sort.
